Invitation to Submit Proposals/Quotation for Accreditation: Carbonated and Non-Carbonated Drinks

°Õ³ó±ðÌýÃÛÌÒ´«Ã½ (PUP), through theÌýResource Generation Office (RGO), invites interested and qualified providers to submitÌýproposals or quotations for accreditationÌýas an Official Supplier ofÌýCarbonated and Non-carbonated BeveragesÌýto be distributed in various concessionaire locations and canteens withinÌýPUP Sta. Mesa, Manila and its Satellite Campuses.

The selected provider shall be responsible for supplying and deliveringÌýquality carbonated and non-carbonated beveragesÌýto support the needs of students, faculty members, administrative personnel, and institutional operations.

A. Legal and Business Requirements

1. ValidÌýBusiness Permit
2. DTI Registration CertificateÌý(for sole proprietorship), or
3. SEC Registration CertificateÌý(for partnership or corporation), or
4. Cooperative Development Authority (CDA) Certificate of RegistrationÌý(if applicable)
5. BIR Certificate of Registration (Form 2303)ÌýandÌýTax Clearance
6. Location Map of the Business Establishment and Warehouse/Distribution Facility
7. Two (2) valid government-issued IDsÌýof the Business Owner or Authorized Representative

B. Regulatory, Sanitary, and Product Compliance Requirements

1. Valid Sanitary PermitÌýissued by the Local Health Office
2. Food and Drug Administration (FDA) License to Operate (LTO)Ìýas Manufacturer, Importer, or Distributor of Food Products (if applicable)
3. FDA Certificate of Product Registration (CPR)Ìýfor beverage products supplied
4. Product Specifications and Ingredient InformationÌýfor beverages offered
5. Description of storage, handling, and distribution proceduresÌýto ensure product quality and safety
6. Photos of storage facility, warehouse, or distribution vehicles
7. Product samples or product catalogue, if required during evaluation

C. Financial and Capability Requirements

1. Latest Audited Financial StatementÌý(signed by a Certified Public Accountant)
2. Bank CertificationÌýattesting financial standing
3. List of major clientsÌý(preferably institutional, commercial, or government clients)
4. Notarized Sworn StatementÌýthat the provider is not blacklisted or barred from any transaction with the government or any of its agencies, offices, corporations, or Local Government Units (LGUs)

D. Detailed Company Proposal

1. Company Profile
   Background of the company, number of years in operation, organizational structure, and operational capacity relevant to beverage supply and distribution.
2. Supply and Delivery Capability
   Description of supply capacity, distribution system, delivery vehicles, logistics arrangements, and service coverage areas.
3. Product and Service Offering
   Types ofÌýcarbonated and non-carbonated beveragesÌýoffered (e.g., soft drinks, juices, iced teas, energy drinks, sports drinks, flavored water), available packaging options (bottles, cans, tetra packs), and proposed delivery schedules.
4. Pricing Proposal
   Detailed product price list, delivery charges (if any), discounts or institutional pricing, and validity period of the quotation.
5. Quality Assurance and Safety Practices
   Description of product quality assurance procedures, storage and handling practices, compliance with FDA and food safety regulations, and product traceability systems.
6. Customer Support and Contact Information
   Designated contact persons, service hotline or support channels, and emergency contact details for operational coordination.
7. Sustainability and Environmental Practices
   Use of eco-friendly packaging, waste reduction initiatives, and participation in recycling or environmental programs.

Terms of Reference for Carbonated and Non-Carbonated Drinks Provider

These Terms of Reference outlines the requirements and responsibilities for the provision of carbo/non-carbonated beverages by the selected provider in various concessionaire locations, canteens in PUP Sta. Mesa, Manila and Campuses.

1. Scope of Services
   1. The Provider shall supplyÌýcarbonated and non-carbonated drinksÌýin various packaging options as required by PUP.
   2. ÌýThe Provider shall ensureÌýtimely and regular deliveryÌýof products to designated locations.
   3. ÌýIf applicable, the Provider shallÌýsupply and maintainÌýany beverage-related equipment such asÌýcoolers, or dispensers.
   4. The Provider shallÌýadhere to all relevant health and safety standardsÌýand conductÌýregular quality testingÌýto ensure product safety and compliance.
2. Service Requirements
   1. Ìý°Õ³ó±ðÌýbeverages provided must meet all applicable health and safety standardsÌýand be free from contaminants.
   2. ÌýThe drinks should be available in aÌývariety of packaging options, including but not limited toÌýbottles, cans, and tetra packs, to suit different distribution needs
   3. ÌýThe Provider shall adhere to the agreedÌýdelivery scheduleÌýand accommodateÌýemergency delivery requestsÌýwhen necessary.
   4. ÌýIf applicable, the Provider shall regularlyÌýservice and maintain any beverage-related equipmentÌýprovided to PUP.
3. Quality Assurance
   1. The Provider must haveÌýall relevant certificationsÌýand comply withÌýnational and local health regulations.
   2. ÌýRegular quality testingÌýof beverages must be conducted, with results available to PUP upon request.
   3. The Provider must comply withÌýall applicable environmental and health regulationsÌýregarding beverage safety, packaging, and distribution.
4. Pricing and Payment
   1. The pricing shall be as per the agreed quotation, with a detailed breakdown provided.
   2. Payments will be made according to the agreed schedule and terms specified in the contract.
   3. Additional costs must be clearly communicated and approved by the PUP prior to being incurred.
5. Cancellation and Termination
   1. Either party may terminate the agreement with a thirty (30) days’ written notice.
   2. PUP reserves the right toÌýimmediately terminate the contractÌýin the event of aÌýserious breach of termsÌýby the Provider.
6. Liability and Indemnity
   1. ÌýThe Provider is liable for any harm caused by the beverages suppliedÌýdue toÌýnegligence or failure to comply with health standards.
   2. ÌýThe Provider agrees to indemnify and hold the PUP harmless from any claims, damages, or expenses arising from the provision of services.
